Every Last Saturday of the Month

Application Requirements for Civil Service Examination
(CS-PPT, PO1 Exam, Stenographer, CSEE)

o Properly accomplished application form(CS form 100 Revised 1999).
o Examination fee stamp to be bought from the cashier’s office and pasted on the spaced provided    in the form:
                            o CS prof/Subprof-PPT P    200.00
                          o CS prof/Subprof-CAT        250.00
o Police Officer Exam        200.00
o Stenographer           200.00
    o Carrer Service Executive Exam
             		       500.00

o four (4) recent identical ID photos (with name tags) pasted on the spaces provided in the form
o Valid identification Card (office ID, Drivers license, PRC ID, Postal ID)
NOTE
         The same ID Card shall be presented on the Examination day.
For Police Officer exam only;
A certified True/Xerox copy of the certificate of Live Birth issued by the Local Civil Registrar of the place where the applicant was born.
